     Your blood glucose monitor, testing strips, lancet devices, and lancets are covered by Medicare if you are being treated with insulin. Private insurance companies cover these and other supplies depending on the carrier. Many individual policies vary and verification is made before shipment.

ACCEPTING ASSIGNMENT

     Accepting Assignment means Diabetes Home Care agrees to charge the insurance-approved fee for a covered service. The primary insurance company will pay us 80% of the approved fee; you pay only the remaining 20% coinsurance. Any deductibles are the customers responsibility. Payment of the deductible can be arranged prior to ordering. The following are the procedures we take in accepting assignment on your covered diabetic supplies:

bulletWe write your physician for a prescription on supplies needed. This is a standard procedure required by Medicare and insurance companies. Your medical information is confidential and Diabetes Home Care will use this data only to process your order and accept assignment.
bulletWe prepare all necessary forms and file the claim to both your primary insurance company and your supplemental insurance.
bulletWe accept payment by Medicare and/or private insurance. (Please note, all deductibles and out-of-pocket expenses are the patient’s responsibility; see your individual policy details.)
